HEALTH
Accident
and Illness Procedure at School
Accidents
School
personnel will take every precaution to avoid accident or injury to the
students. The principal will be notified immediately when a serious
accident
occurs. Parents will be notified of any serious accident, especially
bumps or
blows to the head.
An
injured student will be taken to the school clinic if the nurse or a
clinic
volunteer is on duty. If the clinic is not staffed the injured student
will be
taken to the school office.
School
officials will follow the instructions listed on the student's
Emergency Medical
Form. This form will be sent home in the school packet before school
starts and
will remain on file for the duration of the school
year. Two forms are necessary, one
for the school and one to accompany the student on field trips.
If
any changes occur on this form, please inform the school office
immediately. It
may make a difference should your child become ill or be injured.
Illness
In
case of illness the student will be given a clinic pass and sent
directly to the
clinic or office. The parent will be notified if the student is too ill
to
remain at school. If the parent cannot be contacted, the relative or
neighbor
listed on the Emergency Medical Form will be contacted.
Child
Abuse and Neglect
St.
Albert the Great School adheres to all guidelines for identifying and
reporting
suspected child abuse/neglect as specified in the Archdiocesan Decree
on Child
Protection (previously know as Archdiocesan Decree on Child Abuse.) All
staff
members, coaches, and volunteers are required to attend an in-service
training
session of the decree.
If the staff of St. Albert the Great School documents an occurrence of child abuse and/or neglect, we are obligated by law to make a report to either Montgomery County Children’s Services Board or City of Kettering Police Department. In some cases, both may be notified.
Chronic,
Serious or Altering Medical Conditions
If
there should be a condition that meets these standards, it will be
dealt with by
the faculty and staff of St. Albert the Great School, the parents and
any other
relevant medical experts. A conference is the usual process. Call the
school for
more information.
Immunizations
The
Ohio Department of Health requires the following for school attendance:
The
requirements for students in grades Kindergarten through 12th
grade
are as follows:
| All
Kindergarten students (only) are required to have a fifth dose of
DtaP/DTP/DT |
vaccine if the fourth dose was
administered prior to the fourth birthday.
| All
Kindergarten students (only) are required to have fourth dose of polio
vaccine if the third dose was administered prior to the fourth birthday. | ||
| All students
in Kindergarten through 5th grade must have evidence of
having received two doses of MMR vaccine. This
requirement will continue to raise a grade level each year. | ||
| All students
in Kindergarten through 5th grade are required to have three
doses of hepatitis B vaccine. This
requirement will continue to raise a grade level each year. | ||
| All students in grades 7 through 12 are required to have two doses of MMR vaccine otherwise exempt. |
If
these requirements are not met, the student will be excluded from
school within
14 days of entry date. Exceptions to these requirements are a medical
condition
that restricts these requirements to be met, and/or philosophical or
religious
reasons for these requirements to be met. A letter stating the medical
condition
from the attending doctor or a waiver for religious / philosophical
reasons must
be signed and returned to school in order for the student to be
excluded from
the State mandated regulations. Should an outbreak of these diseases
occur, the
students who have not met the immunization requirements would be
excluded from
school for the duration of the disease outbreak.
Inclement
Weather Conditions
Hot
weather - If the temperature exceeds 80' - 85' Fahrenheit, water
bottles will be
permitted in classroom at all times.
Parents,
please dress your child for the weather conditions. When the weather is
cold,
students are expected to wear coats, hats and gloves because they will
be going
outside for lunch recess. In hot weather, sweatshirts and long sleeved
shirts
are not always appropriate in hot classrooms.
Medical
Conditions
Please
keep the staff and school nurse updated and aware of any medical
conditions that
may manifest themselves during the school year / day. (I.e.: food
allergies,
allergies, asthma, Epi-pen, seizures, diabetes, breathing treatments,
other
treatments, etc.)
In
conjunction with medical conditions, please notify the school nurse and
school
secretaries on updated address changes, phone number changes, and
emergency
contact number changes in order for parent(s) and/or guardian(s) to be
properly
notified of a student illness or emergency.
*Epi-pen,
inhalers, and other medication forms along with the medication given must
be at school by the first day of
school.
Medication
Students
receiving over-the-counter and/or prescription medications (i.e.:
Tylenol, cold
medication, allergy medication, inhaler, breathing treatment, lotions,
oral
medication, ointments, eye drops, etc.) must have a form completed and
signed by
the pediatrician/ family doctor and/or prescribing doctor in order for
the
medication to be dispensed during school hours. The school form must
include:

Parent’s

Sips
& symptoms of side effects and/or reaction
Bottle/container
with MATCHING instructions.
If one
of these items is missing,
the medication will NOT be
given.
If
a change in medication or dosage occurs throughout the school year, a
new form
must be completed and signed in order for the medication to be given.
Prevention
To
prevent the spread of illness between students and especially to the
students
with special medical conditions, please follow these guidelines and
report
conditions to the school nurse and/or school secretaries. A doctor's
note is
required for confirmation of the specific disease. If these
guidelines are
not followed, you will be notified to pick your child up from school.
Chickenpox
- Your child needs to
remain home
until the lost pox has scabbed before admittance to school.
Diarrhea
- Your child needs to
remain at home
for 24 hours after the last episode of diarrhea.
Fever
- Your child needs to
remain at home
until he/she has been afebrile (fever-free) for 24 hours without the
use of
Tylenol/Advil. A 99.5 - 100.0 temperature with additional complaints
will
designate fever/low-grade fever.
Fifth's
Disease - Exclude until
afebrile
(fever-free),
Hand,
Foot, and Mouth Disease - Exclude
until lesions are healed.
Lice/Scabies
- Lice - Nit free for
admittance into
school.
Scabies -
excluded until student and
other contacts (household, etc.) have been treated once.
Pinworm
- Exclude until
adequately treated
under doctors care and no worms present.
Pink
Eye (Conjunctivitis)
- Your child will need to remain at home for 24 hours after
diagnosis and
first dose of eye medication from pediatrician/family doctor. A
doctor's note is
required if diagnosis is not conjunctivitis.
Ringworm
- Exclude student until
24 hours of
appropriate treatment completed. Exclude from sports until lesions are
gone to
prevent spread.
Strep
Throat / Scarlet Fever - Your
child
will need to remain at home for 24 hours after receiving the first dose
of
antibiotics from the pediatrician/family doctor for strep throat. A
doctor's
note is required if diagnosis is not strep thro-at/scarlet fever.
Vomiting - Your child will need to remain at home 24 hours after the last episode of Vomiting.
